For current scholarship recipients, please use the link below to access the online scholarship renewal form.

The Cascade Foundation Educational Scholarship is now available online.  Deadline for submission is due by June 1 each year and is open to individuals with a bleeding disorder (Hemophilia, vWD, or other factor deficiency), or be the sibling or child of an affected individual.  The application can be accessed via the link below.  All applications will be submitted electronically and required documentation can be uploaded as attachments.  If you have any issues or questions please reach out to us at info@cascadefoundationaz.org.
