2021 Hardship Request Program

The Bleeding Disorder Community in Southern Arizona,

Recent events surrounding the COVID-19 pandemic have caused disruption and hardship like we have never seen in our lifetime.  We have always supported the bleeding disorder community through our financial hardship program, however, due to the anticipated increase in financial assistance that will be needed this year we have redirected funds within our budget to expand the hardship program.

Please complete the Cascade Funding Request form and submit to us for review and assistance.  While our goal is to help as many people as we can this year, we are limited in the availability of funds due to our annual fundraiser being cancelled.  We will make every effort to support you and your family through this crisis.

EMERGENCY SPECIAL ASSISTANCE REQUEST FORM - 2021
Submitting this request does not guarantee payment. Approval is at the discretion of the Cascade Foundation of Southern Arizona and is contingent on availability of funds for this program. Assistance is limited to a maximum of $500.00 per household for the year.

Eligibility Guidelines:
1. Applicant must be a resident of Southern Arizona.
2. Applicant must have a diagnosed bleeding disorder or be the parent or guardian of a minor child who lives in the same household who has a diagnosed bleeding disorder.
3. A copy of the outstanding bill or invoice is required and must be included with this application.
4. Applicant must complete all sections of the application thoroughly and accurately and provide corresponding documentation if requested.
